  1. Job Summary:

Within the context of a person centered care model, the Recovery Coach evaluates, plans and facilitates individual and group recovery/rehabilitation programs for adults with mental health and substance use issues.  The Recovery Coach, as a member of an integrated, collaborative team, encourages clients to actively participate in recovery-oriented activities including social, recreational, and leisure activities.       

  1. Role Description/Typical Duties & Responsibilities:
  • Follows the direction of the Program Manager to implement client service based on assessment results and treatment plans;
  • Plans group support programs by determining content and structure by consulting with clients as appropriate, using evidence-based practices of psychosocial rehabilitation and addictions/behavior-change models of practice;
  • Facilitates recovery groups to support skill development and access to resources in the domains of wellness, basic living skills and leisure, employment and education;
  • Evaluates recovery groups using information attained by observation and client feedback on issues such as group session effectiveness, and material presented.  Reports feedback to the supervisor;
  • Participates in case planning with clients and/or healthcare providers by performing duties such as identifying and documenting needs and goals, regular check ins and assisting clients to determine appropriate programs and/or resources.
  • Advocates for clients by problem solving in areas such as landlord-tenant disputes, financial assistance, accessing community resources, and obtaining volunteer placements.
  • Observes clients, identifies and assesses potential emergency situations, and develops short-term strategies to deal with such situations. Reports changes in behavior.  Provides feedback and input regarding clients’ needs, performance, and progress.
  • Provides supportive modeling, demonstrations and behavior management to clients, their families, and their personal networks, to assist with the development of physical, social, emotional, and life skills such as maintaining personal hygiene, housekeeping, meal planning and preparation, meeting financial obligations, making and keeping appointments, and interpersonal skills.
  • Completes and maintains related records and documentation such as group session evaluation summaries, attendance and progress notes
  • Participates in quality improvement activities and implements changes as directed;
  • Performs other related duties as assigned.
  1. Qualifications:

Education, Training and Experience:

  • Bachelor Degree in Human Services, high school diploma and 4 years’ experience working in mental health service provision, or college certificate in human services and 2 years’ experience working in mental health service provision.
  • Unrestricted Class 5 driver’s license, may be required to obtain Class 4
  • Current certificate in First Aid and CPR

Scope and Level Definition:

Assists individuals with mental health and substance use issues to obtain employment within the community by performing activities such as locating prospective employers, facilitating placement of clients, and assisting clients to become job ready. This process includes job search, applying for jobs, resume and cover letter writing, interview practice, symptom management, vocational counselling, and referral to community resources.

Support clients working in the community to maintain their employment and improve their job experience through ongoing support, coaching, and education of client and employer.

Typical Functions and Responsibilities:

  1. Work independently in the community, meeting clients in the community at locations of their choice – 70% of work should be outside of the office.
  2. Interview clients to identify interests, skills, employment and education history, and potential barriers to successful employment.
  3. Refer to other programs and services as required and provide information regarding available community resources.
  4. Locate prospective employers in the community appropriate to clients’ work profiles by following up on referrals, making cold calls, networking, and developing partnerships with government recruiting bodies and employment programs.
  5. Market NetWorks to prospective employers and develop related promotional materials.
  6. Evaluate available employment, match clients in accordance with client work profiles and abilities and market clients to employers to secure employment.
  7. Assist clients to become job ready by providing coaching in employment-related areas.
  8. Provide on-site support to clients.
  9. Follow up with clients and employers to monitor the quality of placements. Assist employers to work with clients by providing education and information and negotiating accommodations, where appropriate.
  10. Ensure close and ongoing communication with case managers and/or other appropriate professionals through regular contact (e.g. attending case and team meetings as agreed) in order to integrate vocational goals into mental health treatment.
  11. Complete and maintains related records and documentation such as reports on clients’ skill levels and progress towards goals and objectives.
  12. Perform other related duties as required.

Qualifications:

Education, Training and Experience:

  • Bachelor’s degree in Health, Social Services, or Career Counselling/Vocational Rehabilitation
  • Recent, related experience of two years
  • Training in Suicide Prevention and Motivational Interviewing
  • Recent experience with substance use issues and harm reduction
  • Recent experience working with clients at risk of homelessness and vulnerable adults

 

Competencies, Skills and Abilities:

  • Familiarity with Individual Placement and Support (IPS) model of Supported Employment
  • Caseload management of at least 15-20 clients
  • Assertive outreach to clients
  • Ensure clients initial appointments are set within one week of referral
  • Knowledge and implementation of psycho-social rehabilitation practice
  • Knowledge of the symptoms of mental illness, with attention to: Schizophrenia, Bipolar Disorder, Anxiety Disorders, Schizoaffective Disorder, and Borderline Personality Disorder
  • Knowledge of substance abuse issues
  • Liaising with mental health professionals and community resources
  • Ability to effectively support clients to establish goals
  • Marketing and promoting clients and the program
  • Client advocacy
  • Ability to work independently, outside of an office environment while maintaining accountability, and managing files and communication requirements

Job Summary:

Provides a liaison role with service providers, case managers, caregivers and other treatment providers.

The Manager is expected to implement new service standards and manage both the staff and program in accordance with all policies and procedures.

This position works 4 days per week, 7 hours per day.

Duties:

  • Contacts/confers with community resources (e.g., family physician or case manager)
  • Plans, develops and implements programming tailored to meet the needs of clients;
  • Conducts intake interviews with caregivers to assess clients in the Alzheimer Support Program;
  • Oversight and management of staff and the day to day operations of the program;
  • Responsible for assigning work, providing feedback and evaluations, determining training requirements, orienting new staff and resolving staffing issues.
  • Completes and maintains related records and documents such as progress reports, statistics, and service logs;
  • Answers general inquiries by telephone or in person, and provides directive and routine information about policies and programs;

Qualifications:
Licensed Practical Nurse, Social Work degree or a Bachelor’s Degree in a field that lends itself to the support of senior care. At least two years of Management/Supervisory experience. Experience with dementia and adult day programming preferred.
